If none of the steps above work then you may have cookies set that can't be deleted or they haven't all been cleared. You can manually clear them from where they are stored:

If you have cookie files that say they can't be removed (particularly index.dat) then you can do one of two things:
1. Use a tool such as
move on boot which is a free utility that allows you to mark a file for deletion at the next boot
2. Use a privacy tool - there are plenty of them about - the best free one is probably
MiL Internet Eraser or if you don't mind paying then
CC Cleaner is pretty robust as is the paid version of MiL.
It is probably good practice to use one of these tools anyway - the index.dat file stored by IE does a lot more than most people realise:
Index.dat is a file used by Microsoft Internet Explorer to store web addresses, search queries, and recently opened files - every single picture, video, websites you typed in the url box, and even emails that you have sent or received are also being logged.
Whichever method you use this should remove all of the cookie data from your computer and ensure the older AFF cookies is gone.
